GENERAL SUMMARY:
Responsible for providing Case Management and Counseling services for program participants and employees of GICW as assigned.
ESSENTIAL FUNCTIONS:
- Conduct behavioral and vocational assessments based on referral information; develop program goals and Individual Plans according to the needs and preferences of each assigned individual.
- Provide crisis intervention; individual, family and group counseling; advocacy skills training.
- Maintain visitation schedule for all assigned worksites.
- Maintain individual files on individuals served.
- Maintain confidentiality of records.
- Assist individuals with financial/work matters, including Social Security matters.
- Keep accurate case notes and case records. Generate reports as required.
- Ensure appropriate information is entered into the departmental databases.
- Create and maintain departmental forms.
- Comply with all state and county regulations.
- Maintain certifications as required
- Meet with Director of HR (and other departmental directors as appropriate) to assess individuals progress.
- Attend informational meetings held by other agencies as assigned.
- Assist Director and Senior Program Manager with maintenance and development of Standard Operating Procedures (SOP’s), CARF preparation and other reports as assigned.
- Assist the department with meeting annual Business Plan goals.
- Comply with all GICW policies and procedures, CARF standards, Safety and security regulations, and HIPPA regulations.
- Coordinate team approach and chair meetings for development and implementation of treatment plan/goals for each participant/employee on case load.
- Complete and maintain accurate and timely reports and other forms of documentation.
Job Requirements
QUALIFICATIONS:
Specific:
- Must be 18 years of age or older.
- Master’s Degree in Rehabilitation Counseling required/CRC Certification.
- Required Certification as a Certified Rehabilitation Counselor (CRC) by the Commission on Rehabilitation Counselor Certification
- Must have at least two years counseling experience.
- Experience with windows environment computer systems including Word and Outlook.
- Time management skills.
- Must be detail oriented; multi-tasking required.
- Must be able to function independently and in a team environment.
- Must have excellent written and verbal communication.
- Must be able to provide own transportation and travel within the service area as assigned.
General:
- Must maintain consistent, predictable and satisfactory attendance and punctuality as scheduled.
- Must be able to demonstrate upon request functional literacy and numeric proficiency.
- Must have command of the English language sufficient to verbally communicate with the general public.
- Must not present a significant current risk of substantial harm to self or others in the performance of the essential duties of the job that cannot be eliminated or reduced by reasonable accommodation.
- Must provide proof of identification and eligibility to work in the United States of America.
