Description HOSPICE-NURSING        
SouthernCare Inc., one of the nation's largest hospice companies, is seeking:

• RN-Case Manager
   Current nursing license required.


• Hospice Aide - CNA

• Registered Dietician


for our local CHARLESTON, SC office.
Requirements
• RN-Case Manager
The Hospice RN assists in the development and supervision of the Plan of Care for each of their patients; they visit their patients on a regular basis; perform physical assessments; monitor medications; work closely with the physician & other team members to manage pain and other symptoms; educate & provide compassionate support; help make every day count.  Many nurses who have worked in other settings say that hospice allows them to spend quality time with their patients and families, develop caring relationships and enables them to utilize a great deal more of their professional training.  Must have reliable transportation.

• Hospice Aide - CNA
The Hospice Aide assists with the patient’s personal care and helps the patient and family with activities of daily living – including light meal preparation and light housekeeping.  They also provide companionship and valuable emotional support to the patient and family.  They free-up family caregivers, allowing them the opportunity to focus on spending quality time with their loved one. Must have reliable transportation.  Home care/hospice experience is preferred,  however training is available for those with no experience.

• Registered Dietician
The Registered Dietician provides dietetic and nutrition assessment and services to our patients. This individual will work closely with the Clinical Director, participate in hospice’s assigned quality assurance and risk management program activities as needed. Must be licensed in the state of practice with a minimum of one year of professional dietetics experience preferred.
