Description
A Touch of Grace Hospice is looking for psychosocial professionals (Licensed Social Worker or a Chaplain) for a part-time position with capability of growing into a fulltime position

Our mission is to:
• Provide culturally sensitive hospice care to patients and families
• Promote patient/family choice in quality comfort care
• Provide excellent social, medical, and spiritual support to patients and families
• Never compromise our, nor the family’s integrity while caring for the patient
• Promote living and dying with dignity
• Support the well being of our staff, so they in-turn will promote the well-being of patients and families.

We are currently looking for a psychosocial professional who will evaluate patient/family social needs which include assessment of; environmental, religious, financial, psychosocial, caregiver(s), special activity and psychological needs.

 

DUTIES AND RESPONSIBILITIES:

1.     Complete an initial psychosocial assessment for hospice patient/families.

2.     Provide counseling to patient and caregiver(s) as needed.

3.     Make community referrals and recommendations to help meet the social needs of the patient/caregiver(s)

4.     Attend and participate in weekly interdisciplinary team meetings.

5.     Serve on hospice committees as requested by management team.

6.     Complete all necessary paperwork in a timely and efficient manner

Requirements

SOCIAL WORKER QUALIFICATIONS:

1.     A commitment to the hospice philosophy of care.

2.     Masters Degree in Social Work from an accredited school.

3.     Licensed as a Social Worker in the state of Tennessee

4.     One (1) year of experience as a Masters level Social Worker in a health care setting,    previous hospice experience preferred.

5.     Exceptional communication skills, oral and written.

6.     Valid Tennessee drivers license and automobile insurance

  

CHAPLAIN QUALIFICATIONS:

1.     A commitment to the hospice philosophy of care

2.     Bachelors or Masters in Divinity or other related field

3.     One year experience as spiritual counselor, preferably in a health care setting

4.     Exceptional communication skills, oral and written.

5.     Valid Tennessee drivers license and automobile insurance
