Job Description 

RN CASE MANAGER

 

A non-profit Hospital in Luzerne County is looking for a Case Manager.  In this position you will be responsible for providing skilled nursing services and management of client cases as assigned;

 Participate in establishment and evaluation of nursing care plan. You will assures accurate OASIS completion for appropriate home health clients within twenty-four (24) hours of admission, recertification, resumption, or discharge. You will see Hospice patients in their home and keep their family informed.  You will also offer bereavement to family members.  Hospice experience is preferred but not mandatory.

  • Graduate of a school of nursing with current license(s) to practice as

                a nurse in Pennsylvnaia

  • Current CPR certification within three (3) months of employment.

  • Hospice experience is preferred but not mandatory

  • A minimum of one (1) year of clinical nursing experience -
