Description
POSITION PURPOSE:
To support the Medical & Quality Management (MQM) department initiatives through the coordination, education, clinical management support and monitoring of members participating in a Patient Centered Medical Home (PCMH) and to maintain or improve their health status across the continuum. Identify and stratify potential members for the medical home. Evaluate the pilot program process and make appropriate adjustments to meet the goals of the program. Track project outcomes based on the specific goals of the program
Requirements
MINIMUM POSITION REQUIREMENTS:
Required Experience:
· 5 years clinical experience in a physician/provider office or outpatient setting with emphasis on coordinating patient care and patient education
Required Certifications/Licenses:
· Valid RN license
Required Knowledge, Skills and Abilities (KSAs):
Knowledge of:
· Managed care delivery systems
· Group process and team dynamics
· CPT-4, DRG and ICD-9 coding
· CMS standards for Medicare Advantage Plans
