MINIMUM POSITION REQUIREMENTS:

Required Experience:

·         5 years clinical experience in a physician/provider office or outpatient setting with emphasis on coordinating patient care and patient education

Required Certifications/Licenses: 

·         Valid RN license

Required Knowledge, Skills and Abilities (KSAs):

Knowledge of:

·         Managed care delivery systems

·         Group process and team dynamics

·         CPT-4, DRG and ICD-9 coding

·         CMS standards for Medicare Advantage Plans

·         Verbal and written communication

·         Microsoft Word – Intermediate

·         Perform initial telephone screening from physician office

·         Evaluate and stratify member population

·         Schedule time effectively

·         Handle multiple projects simultaneously

·         Prioritize work appropriately

·         Identify members requiring case/disease management

·         Provide short term case management
