Min Qualifications
Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science.
JOB DESCRIPTION:
This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.
Salary Range
From $26,816.00 To $33,520.00 Annually
Specific Job Related Duties
32 work hours per week
Maintains regular contact with each assigned patient to monitor responses to services and identify problem areas.
Obtains information from family members about patient in regard to past leisure activities; reports significant information and makes recommendations to the treatment team members.
Liaison with MHMR and local social service agencies to develop appropriate referrals to assist the patient to return to his or her community upon discharge.
Provides group and individual counseling as assigned, to prepare patients' or clients' with skills necessary for successful adjustment beyond the scope of services received in the telepsychiatry program.
Documents case records as needed.
Performs related duties as required.
Preferred Education
Master's degree in social work, counseling, or psychology from an accredited college or university.
Preferred Language
Bilingual in Spanish and English speaking skills.
Preferred Skill/Competency
Ability to communicate professionally and effectively, both orally and in writing, with patient families, patient advocates, and agency officials.
Ability to gather and synthesize information to be utilized in the coordination of patient care and treatment planning.
Knowledge of social work values, principles, and methods.
Knowledge of group dynamics and professional personnel interaction.
Knowledge of case management principles and terminology.
Skill in conducting social and development history assessments and evaluations.
Knowledge of continuity of care process and hierarchy of services available and how to access them.
Preferred Work Experience
Experience in mental health setting.
