RN - HEMOPHILIA TREATMENT CENTER PHARMACY
Skilled Nursing Coordination:
Contract with and arrange for home infusion therapy with home health agencies, generating authorizations at contracted rates for in-home skilled nursing services. Review of nursing notes prior to payment of invoices for services.
Collaborate with the patient/family and interdisciplinary health care team members in planning, implementing, and evaluating treatment regimen.
Provide professional nursing care and treatment of adult and pediatric home infusion patients in accordance with the prescribed plan of care and with company policies and procedures, all applicable accreditation, federal, state, and local regulations.
Apply appropriate ethical, legal, confidentiality and advocacy principles to the planning and delivery of patient care. Ensure correct, appropriate and timely physician orders for the provision of nursing care in the home setting.
Develop and maintain an active, dynamic database of preferred home health agencies.
Identify learning needs of patient and/or caregiver in the home setting. Provide appropriate intervention and instruction for patient/caregiver to safely administer care at home.
Train patients and families in self-administration of intravenous treatments, and how to recognize and respond to adverse reactions.
Ensure patient competence and compliance with all self-care, including venous access, central line procedures and infection control; re-educate as necessary, and document/communicate the need for re-education with hemophilia center staff.
Provide consultation & education to schools, hospitals, and other community resources that may provide services to patients with hemophilia.
Prepare clinical and progress notes according to assessment of the patient.
Appropriate documentation of patient care activity, provision of home infusion nursing services and all communication with patient/family, HTC clinical staff and HTCP staff. Documentation must be legible, accurate, complete, and compliant with and procedure for electronic and paper patient clinical record entries.
Maintain HTCP patient files according to policies and standards set by regulatory and accrediting agencies of the HTCP.
Provide or arrange for essential in-home IV infusion medical treatment, response management and follow-up care to clients. This is therefore a multi-faceted role of nurse, educator, and coordinator.
Experience with peripheral and central line access; infusion experience with both adult and pediatric patients. Experience performing venipuncture and experience with central catheters and PICC lines.
Demonstrate competency in venous access device management, infection control, and safe practice of infusion nursing.
Participate in continuing education and training related to bleeding disorders and infusion therapy services as required by the state board of nursing.
Develop a working knowledge of public and private health care reimbursement systems and the Affordable Care Act.
BSN required, MSN preferred. Position requires graduation from an accredited school of professional nursing and an RN license through the NC Board of Nursing with a minimum of 3-5 years clinical nursing experience to include pediatrics.
Strong venous access skills; PICC and/or CRNI certification preferred
Basic computer skills including Microsoft Office, Internet and email
Must possess a valid and current driver’s license and have car available for work use; auto insurance
Requires graduation from a state-accredited school of professional nursing. Must be licensed to practice as a registered nurse in the state of North Carolina by the NC Board of Nursing. Must have one year of experience as a nurse.
