Clinically based practice includes medication order review, renal dosing, pharmacokinetic dosing and monitoring, medication reconciliation, disease state therapy adjustments and oversight of adverse reactions. Practice setting includes: *central and decentralized work stations; *advanced information and computer technology for order entry, dispensing and medication administration; *a rapidly expanding, state of the art facility. BS in Pharmacy or Pharm.D degree required. Applicant must have current IN Pharmacist license. Previous hospital or clinical experience preferred.
