Job Description

Our Connecticut adult services division is currently seeking a 20-hour Nursing & Health Care Coordinator to provide case management, staff training and on-call consulting to adults with developmental disabilities. This position includes a flexible schedule and will be assisting houses in Niantic, Groton, Pawcatuck, Norwich, and Mystic.


Responsibilities:

  • Provide health & IP assessments
  • Supervise the Med Administration Program for initial Pass & Pours, nursing delegation, med errors and protocol oversight at each house
  • Provide health education to staff at the house
  • Oversee maintenance of individual medical records.
  • Oversee health care and dietary issues of residents
  • Coordinate medical services with other providers
  • Participate in on-call rotation for after-hours


  • Job Requirements

    Requirements:

  • Current CT RN license
  • Strong computer skills preferred
  • Must possess great communication skills with ability to work independently
  • Experience with MR/DD strongly preferred
