World Renowned, Long Term Care Facilities in the Bronx and Manhattan are looking for a Clinical Admissions Specialist and Admission Liaison Nurses.

 

Salaries up to $90,000 with excellent benefits!

 

Clinical Admissions Specialist - Manhattan

MAIN FUNCTION:

Responsible for reviewing PRI’s and other supportive documentation, evaluation for appropriateness of  admission and the coordination of the admission of residents to the facility in a manner that is supportive of the Home's Mission and Values.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Collaborates with patient or representative and interdisciplinary team to ensure seamless transfer to the facility, including the discharge plan.
  • Acts as the single point of decision making for admissions, collaboration as indicated.
  • Completes a pre-admission assessment identifying medical, social issues and financial coverage in conjunction with the Admissions Manager.
  • Analyzes the resident needs and coordinates with facility services to plan for an appropriate facility placement.
  • Regularly reviews the master discharge schedule.
  • Participates in quality improvement processes.
  • Conduct on-site visits as needed at the acute care setting and resident home.
  • Acts as an initial contact for discharge and transfer of patients from area hospitals and other care settings to the facility.
  • Make rounds with case managers and or other hospital staff as requested.

 

Admissions Liaison (LTC) - Bronx

This position will be based in the Nursing Home, but will be required to travel. This is a Monday - Friday position but might have a weekend rotation once every other month.
Duties

  • Provides direct linkages between the nursing home and their divisions as well as general health care community in order to identify and assess potential applicants and to ensure timely admissions and placements.
  • Participates in the pre-admission screening process by collaborating with discharge planners form various healthcare facilities.
  • Visits residents of the facility who are hospitalized in order to Assess their needs upon return from the hospital as well as act as an advocate and provide psychosocial support for hospitalized residents.
  • Identify and determine potential residents/clients who meet clinical admissions.
  • Screens and reviews prospective residents clinical background, physical and psychosocial needs in various healthcare settings.
  • Reviews the medical, record, interviews potential residents/families and confers with hospital/rehab Medical and social work staff as part of the screening process.
  • Maintains a computerized listing of all potential applicants evaluated for admissions to a facility or placement in a program.
  • Performs marketing and outreach activities to disseminate information about all divisions of the facility and the admissions process.

 



Job Requirements

  • Must Currently have a NY State RN License.
  • Working knowledge of long Term Care programs, services and levels of care.
  • Three years of clinical nursing experience in a Long Term Care setting or in a field of aging.
  • A valid Drivers License and BSN required.
  • Organizational Skills and strong physical assessment skills.
  • Knowledge of computers and software.
