Social Worker - LICSW Required

• Provide emotional support and counseling to Individuals and families from intake to           discharge through personalized visits.

• Maintain a minimum Productivity Standard according to Allegiance policy.  Productive time consists of face-to-face contact with Individuals and families in the form of direct care activities.  Productive time also includes coordination of care activities with families, physicians, and host nursing facility partners, such as:  TQMs, at-risk meetings, information presentations, or meet and greets.

• Perform initial assessment of Individuals’ level of care needs and develop psychosocial recommendations for the Plan of Care.  Present Plan of Care recommendations to interdisciplinary (IDT) team.  Develop a comprehensive psychosocial plan based on the IDT assessment.

• Assess hospice Individual/family factors to estimate capacity to cope with terminal illness and death.  Provide counseling services to Individual and family per Allegiance policies.
