You will be part of a team of compassionate and dedicated nurses, nurse assistants, chaplains and others focused on improving the quality of life of the hospice patient by easing the emotional, spiritual and physical pain often associated with a terminal illness. 

RN Case Manager Job Duties:

• In conjunction with the Interdisciplinary Team (IDT), manage and coordinate the Individual and family/caregiver Plan of Care from referral to discharge.
• Facilitate communication about changes in the Individual’s status among IDT members and the Individual’s attending physician.
• Apply the policies and procedures of Allegiance and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in providing hospice care.
• Plan, coordinate, and deliver direct nursing care to Individuals and families according to the Individual’s need.
• Visit each Individual at least every 14 days in accordance with regulatory requirements.
• Perform a comprehensive assessment of the Individual/family and develops a Plan of Care and ensures the care plan is up to date and reviewed at every team meeting.

• Current license as a Registered Nurse (RN) in good standing with the State Board.
