POSITION: Wound and Ostomy Nurse/Certified Wound Nurse/ Wound Care Certified
POSITION SUMMARY:
- Responsible for providing skilled nursing care to patients with ostomies, wounds and fistulae.
- Using the nursing process and the expertise of wound and/or ostomy nursing, coordinates the total rehabilitative needs of the patient, including coordination with other team members.
- The Wound and/or Wound/Ostomy Nurse/ Wound Care certified instructs the patient, caregiver, nurses, physicians and other health care professionals in the care of patients with ostomies, wounds and fistulae.
- Documents observations and interventions, participates in quality management activities and performs related supply management.
- Participate in Wound and Ostomy committee.
- A Wound Care certified (WCC) nurse works with a nurse certified by Wound, Ostomy, and Continence Nursing Certification Board.
AREAS OF RESPONSIBILITY:
- Admits patients to service and documents skilled nursing care, procedures and education of the patient and family.
- Promotes comprehensive assessment of the patient’s environment including physical, mental and social status and eligibility for home care services in accordance with third party payer regulations.
- Assists physicians in caring for patients with stomas, wounds, fistulae, and incontinence. Skills include conservative sharp debridement ( must have certification with HR before allow to do), wound care management, teaching patient /caregiver woundcare, stoma sighting, teaching intermittent self-catheterization and consultation.
- Serves as wound/ostomy ( if within specialty) consultant to the nursing staff, physicians and other health care team members.
- Attends staff team meetings as required, communicates the patient’s special needs to appropriate health care team members. Reinforcing with staff wound/ostomy policy of agency.
- Orients new nursing personnel and selected nursing students to the role of the Wound/Ostomy Nurse.
- Assist with doing in field check-offs as needed for VAC or compression therapy.
- Develops and monitors standards of care, assists in the formulation and evaluation of agency procedures, protocols and policies as requested with Wound/Ostomy Care Program Manager.
- Evaluates, selects and recommends supplies and equipment for patients with ostomies, draining wounds, fistulae, pressure ulcers and incontinence using Pathways’ Formulary.
- Responsible for professional development relevant to Wound/Ostomy nursing services, including membership in associations and attendance at conferences.
- Performs other duties as assigned consistent with the skills and training and the goals and mission of the Agency.
Requirements
Wound and Ostomy Nurse/Certified Wound Nurse/ Wound Care Certified
QUALIFICATIONS:
- Registered Nurse licensed in the State of California.
- Satisfactory completion of an accredited WOCNEP school accredited by the Wound, Ostomy and Continence Nurses Society (WOCN) or IAET accredited ET Nursing Education Program or National Alliance of Wound Care (provided for WCC – Wound Care Certified with supervision of CWOCN.
- Current Board Certification in ET Nursing from the ET Nursing Certification Board (ETNCB); Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB); National Alliance of Wound Care ( provided for WCC – Wound Care Certified)
- Minimum of two years of experience in acute care setting.
- Minimum of one year recent home care experience preferred.
- Current California Drivers License and Automobile insurance.
- Current CPR Certificate.
