Description

RN Liason Job Duties:

Identify and develop ongoing contacts with specific acute care hospitals, long-term care facilities, and assisted living facilities to maintain relationships for the promotion of public awareness and understanding of the hospice program and philosophy.
Screen Individuals for admission to Allegiance.
Maintain a minimum productivity standard according to Allegiance policy, generated from an appropriate volume of inquiries and referrals.
Provide clinical contact with Individuals/families to promote options for end-of-life care.
Visit Individuals and families in the acute or long-term care facilities per Allegiance policy.
Serve as the liaison between referral sources and the Allegiance interdisciplinary team (IDT).
Complete admission paperwork as requested completely, accurately, and timely.
Provide in-services to community agencies, physicians, and hospital referral sources in support of hospice.

Requirements

Current license as a Registered Nurse (RN) in good standing with the State Board.
Must have demonstrable competency with computers (MS Word, MS Excel, etc.)
Candidates must successfully complete criminal background check and physical(including drug test & TB screening) prior to beginning orientation.
