CLINCAL NURSE MANAGER - HOME HEALTH

Private Home Care, Home Staff, Inc. and Pri-Med Infusion are owned and operated by an experienced home care nurse for over 20 years.

Home care nursing is a fast-growing professional career of compassionate, one-on-one nursing care provided in the home setting.  Nurses providing home care are the case manager of the patient.  Because patients are being discharged from the hospital quickly, the demand for quality nursing care in lieu of the patient being hospitalized is creating a larger demand on the skilled nurse to meet the tremendous growth. 

The Clinical Nurse Manager is responsible for ensuring that patient care is coordinated and managed appropriately.  Ensuring the care and services are delivered appropriately as well as the supervision of clinical personnel.

Our nurses are matched with the appropriate patient based on the nurses’ competency assessment, geographical area the nurse is available to travel to treat the home bound patient and the needs of the patient. 

GENERAL JOB DUTIES

1. Receives case referrals. 
2. Reviews available patient information related to case, including disciplines required, to determine home care needs.  Assigns appropriate clinicians to case.
3. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
4. Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

1. Registered nurse with current licensure to practice professional nursing in the State of Illinois

2. 3 years recent management or supervisory experience

3. 3 years recent clinical experience in a home care environment,

4. Excellent clinical skills, expert knowledge of infusion care

5. Current CPR certification

6. Demonstrates excellent observation, verbal and written communication skills
