    Position Summary:

    Responsible for assessment of patient/ family psychosocial needs and coordination of the individualized psychosocial plan of care in the home, nursing home, or inpatient hospice setting.  Provides psychosocial support, counseling, and resource planning in accordance with the plan of care.  Serves as a resource for the hospice team, organization, and community regarding psychosocial aspects of end of life care.  Participates in professional development activities that further personal and program goals related to end of life care.
    Recruitment Requirements: 
  • Masters degree from an accredited school of social work
  • Current licensure in PA and/or NJ
  • One to two years post graduate degree experience in a health care setting required
  • One year working with terminally ill patients preferred
  • Ability to work within an interdisciplinary team
