• At least one year experience in as a registered nurse / rn In specialty 
• Currently licensed as a registered nurse  in New York without Restrictions
• Eligibility to work as a registered nurse in the United States is a must
• Graduate of an accredited registered nursing program
• 12 or more months of  experience as a registered nurse in specialty.
• Current BLS certification

•  ACLS-AHA – required for all Critical Care, Step-down, Recovery Room, PACU and ER

   units

• CPR-AHA – Must be healthcare provider level course, American Heart Association only

• PALS-AHA – required for ER, PICU, PEDS

• NRP – required for L & D, NNICU, Post Partum, Ante-Partum

• Criminal Background Check/Terriost Background Check – Past 7 years, every county lived and worked must be run

• 10 Panel Drug Screen – must be completed within 30 days prior to start date

• Color Blind Exam ( completed online )

• Photo ID

• Social Security Card

• Unit Specific Skills Checklist – must be updated annually

• 2 References/ performance evaluations

• OIG

• GSA

• Hepa B Declination Form

• Specialty ( Unit ) Exams ( online ) – varies per specialty (please see instruction letter, marked with an “x" )

• Physical – completed within one year of start date

• 2 Step PPD – completed within one year of start date ( positive PPD’s must be accompanied with a Chest X Ray and TB Signs and Symptoms form completed within one year of start date) 2^nd PPD to be placed 30 days within start date.

• Rubella, Rubeola, Varicella, and Mumps Titres – lab forms accepted only, handwritten titres not accepted, if titres come back negative, must get two vaccines, with titre redraw

• NY License & Online Verification
