Description The Patient Care Facilitator RN serves as a clinical resource to agency staff on issues pertaining to patients with wounds, pressure ulcers, ostomies, fistulas and incontinence.

Serves as a resource person for WOC patient care practices and makes home visits as needed.
Works with the Clinical Educator/Facilitator on the development of education programs specific to Wound Ostomy Continence patient care.
Functions as Home Health liaison to Wound Ostomy Continence Program at St. John's Hospital, acute care setting.
Develops, facilitates and coordinates a cost effective wound/ostomy care program in both Springfield and Decatur offices

Requirements

Registered Nurse with a current Illinois license. Bachelor’s degree in Nursing required, Master’s preferred. Completion of an accredited Wound Ostomy Continence ET Nurse Program and current Certification in Wound Ostomy Continence Nursing.
Must have 2-4 years of broad base medical / surgical nursing experience and 2-4 years of wound care experience, at least 1 year of home health experience, preferred; teaching experience preferred.

Must possess a valid Illinois drivers license and have automobile insurance. Must provide own vehicle for transportation for visits
