You’re exactly who we’re looking for at Lexington Health and Rehab, a 154 bed Greystone Healthcare managed facility!  We have a rewarding career opportunity for an Admissions Assistant.  The Admissions Assistant coordinates patient admissions with physicians, hospitals, attorneys, social workers and families to provide continuum of care for patients/residents.

This position acts as a liaison between the facility and the outside community and develops strategies to maximize admissions of patients requiring a sub-acute level of care.

Responsibilities include:

  • Maintaining daily contact with potential referral sources including hospitals, physicians, insurers, case management companies and health care agencies.
  • Serving as a resource person for patient care planning activities.
  • Participating in direct and indirect patient care activities as well as quality improvement activities which reduce cost and increase efficiency through improved systems and processes.
  • Actively working with local community agencies to generate a positive image and encourage word-of-mouth referral activities.

  • Requirements
    • Minimum one year experience in a skilled nursing facility or long-term care admissions.
    • Must be proficent in writing reports, business correspondence and policy procedure manuals.
    • Demonstrate ability to function independently and possess strong leadership skills.Establish and maintain positive relations with residents, families, staff and visitors.
    • Knowledge of regulatory standards and compliance requirements.
