Description The Physical Therapist implements and coordinates client physical therapy services that comply with professional therapy standards and physician orders.

JOB DUTIES:
1. Assess and develop a program for therapies that meet individual client needs.
2. Implement the client's plan of care for therapy.
3. Coordinate therapy modifications to the client's plan of care with the Clinical Supervisor and client's physician.
4. Ensure effective coordination of client home care services through timely completion of required documentation and transfer of pertinent medical data to the clients physician, Clinical Supervisor, and other caregivers.

Requirements EDUCATION/QUALIFICATIONS:
Must be a graduate of an accredited school of Physical Therapy, be licensed by the State of Wisconsin as a Physical Therapist (PT), with a minimum of one-year experience in physical therapy. Excellent oral, written, and interpersonal communication skills as well as organizational and time management skills are required.

SCHEDULE:
Full time or Part time. Weekdays. Paid per visit. Flexible due to the changing needs of the clients served.

ENVIRONMENT AND LIFTING REQUIREMENTS:
Must be able to lift, turn, and transfer up to 30 lbs.
