As a Rehab Program Coordinator you will be responsible for an interdisciplinary team.  This will include working closely with our District Manager with regards to:  daily staffing and management, participation in facility meetings, planning RUG & PCT levels and month end operational reports.  You will be intertwined in the scope of practice; ensuring the skills of your team by facilitating training programs and building new specialty programs.  You will be encouraged to establish student affiliation relationships with local schools and will have the ability to take on additional extracurricular activities, as your interest warrants.  We are looking for a therapist with strong communication skills to build relationships and bridge communication gaps with nursing and administration.  Previous experience in a Medicare environment is a must – PPS, RUG levels, documentation requirements, patient screening …  This is a terrific opportunity to either hone your skills as a manager to take that next step in your career.  Check out our web site for additional information on our training programs, career tracks and master clinician certifications.

 

As an “RPC" <b>you will need to have graduated from an accredited therapy program in either Physical, Occupational or Speech Therapy and hold a state license<b/> in one of these professions, as you could also be treating patients 25-50% of the time.

Join a supportive multidisciplinary team of therapists in this 92 bed eldercare setting.  Avoid all the congestion of the large city - Farmington is just a short & beautiful drive in the country from Peoria.  As we build and expand our functional based rehab programs, we are looking for an experienced therapist to add to our team.  While it is a smaller facility, we achieve big results with our patients.  We have a new Modalities program here – complete with lots of equipment for you.  The DON is very supportive of therapy and ensures a supportive environment with the nursing team.  We have a pretty good mix in the case load with Med As & Med Bs.  We are eager to talk to you about the caseload and potential flex scheduling options.  We will consider a therapist of any discipline for this position, with previous experience in a leadership role.  Call now to begin the process.

 

 

Requirements
Performs evaluations, develops and implements effective patient treatment plans to restore, compensate or adapt for loss of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing the facility and their discipline.

Qualifications
• Graduate from an accredited program for Physical Therapy.
• Must possess and maintain a current license in the state of practice as required.
• Must possess problem-solving skills of the type and at a level necessary to accomplish the job.
• Must demonstrate good time management and organizational skills.
• Must be able to communicate in English, both verbally and in writing.
• Must be able to relate professionally, positively, and cooperatively with patients, patient’s family members and facility staff.
• Must be capable of maintaining regular attendance.
• Must meet all local health regulations, pass post-offer drug test, and post-offer employment physical exam if required.
• Must be capable of performing essential job functions of this job, with or without reasonable accommodations.
• Must have own transportation to travel between sites as needed.
• Must possess computer skills of the type and at a level necessary to accomplish specific job duties and assignments.
• Must meet Clinical Competency requirements upon hire and as needed.
