SERVICE COORDINATORS
KenCrest Services is looking for two (2) full time Service Coordinators to support its new Birth to Three Home Community Program in and around Wilmington, Delaware.
The Service Coordinator provides service coordination to families who have children with identified developmental delays.
Responsibilities include:
· Assist with the development of the individualized Family Service Plan
· Facilitate a team based approach to early intervention supports and services
· Help families gain access to needed supports and service
· Continually monitor the child/children’s developmental needs and appropriateness of services
· Assist with the transition to school services as needed at age three
· Provide interim early education services in the home or community
· Provide ongoing consultation regarding child/children’s development in home/community setting
Requirements
- Bachelor’s Degree in Early Childhood Education, Special Education or General Education
- Three years of experience providing Early Childhood Education to infants and toddlers
