• Home Care and Hospice conducts over 45,000 home health visits per year, is the only Home Health and Hospice facility in the area accredited by the Joint Commission, and offers a full range of services that ensures smooth transitions.

Position Information

• The Director will be responsible for the oversight of the hospice and home health departments with 98 FTEs and 8 direct reports.  This department serves more than 200 patients in a 9 county region.
• Qualified candidates will be bachelor’s prepared professionals who have home health and hospice leadership experience, strong relationship building skills and an enthusiastic approach to their work.  Master’s preferred.
• Ideally, candidates will have proven ability to grow this service line, the ability to increase accountability and clinical strength within the department and to mentor and develop the employees.
