Description
SUMMARY
Provides counsel and services to individuals and families requiring social services assistance by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Interviews clients and enters intake data into client case management software program at the time of the interview.
Determines client's eligibility for assistance, and provides services as appropriate.
Provides Outreach Services, including home visits, as directed by Community Services Director.
Keeps and updates community resource information, and refers clients to appropriate resources and other organizations as needed.
Maintains case records on all clients.
Administers the Comprehensive Family Assessment Tool to assigned Family Self-Sufficiency (FSS) clients, and others as directed; scores and records baseline benchmarks.
Aggregates Outcome Scales into an individualized Family Development Matrix and uses this tool to monitor and measure FSS client progress.
Using the Logic Model, develops with FSS clients an individualized service plan.
Meets regularly with FSS clients to monitor progress of Service Plan goals and objectives. Helps client modify Service Plan as needed.
Prepares and submits to Community Services Director a monthly case management report, and compiles additional data as directed. Notifies the Community Services Director immediately of any significant concern or event relating to any client.
Accesses and records client and community resource information.

EDUCATION and/or EXPERIENCE Bachelor Degree from an accredited college or university and one (1) year related experience. Associate's degree or equivalent from two-year college or technical school; and two (2) years related experience and/or training; or equivalent combination of education and experience.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
COMPUTER SKILLS Knowledge of computer system. Ability to enter and retrieve data using computer system, system applications and other office equipment.
MATHEMATICAL SKILLS Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Valid Florida Driver License
