Description
Clovis Community Medical Center is a top choice for minimally invasive surgeries, women’s services, sports medicine, screenings and diagnostic procedures. In 2007, 2008 and 2009, Clovis Community was recognized as the top bariatric surgery center in all of California and has long been a top choice for expectant mothers in the region. Along with a full spectrum of acute-care services, Clovis Community offers the area’s premier breast care center and a comprehensive outpatient care facility.
The 109 bed Clovis Community Medical Center, is expanding to meet the need of its growing region and recently completed a 22,500-square-foot addition and renovation to the outpatient care center. It includes the most advanced (Level 3) fertility center between Los Angeles and Sacramento, expansion of the Marjorie E. Radin Breast Care Center as well as new operating rooms and a dedicated endoscopy center.
Further plans include the largest-ever expansion on the Clovis campus– a $285 million project that will make Clovis Community the first comprehensive, full-service hospital in the region to have all private rooms, nearly tripling in size to 739,000 square feet. Groundbreaking is set for early 2010 and expansion and renovations over the next few years will include a new, five-story bed tower, a dedicated women’s pavilion, a special care nursery and a new parking structure. The emergency department will also expand to accommodate a projected 50,000 patient visits annually.
JOB SUMMARY
In this role, you will provide the delivery of basic pharmacy services, patient-oriented clinical pharmacy services, drug information and monitoring. You will also review all drug orders and check all work completed by ancillary personnel in the department. You will serve as a drug expert and consultant to physicians, nursing staff and other hospital personnel. In addition, you will provide quality assurance review of drug utilization throughout the hospital for various medical staff committees and the department.
ESSENTIAL ACCOUNTABILITIES

Provide consultation services at point of service: Bedside and chart assessment of patients, performs appropriate calculations and provides recommendations on drug therapy and dose.

Verifies accuracy of medication compounded and dispensed from Pharmacy Services.

Provides drug information to medical, nursing, and/or ancillary staff as needed.

Prepares intravenous additive solutions and extemporaneously compounded medications accurately.

Interprets physician orders and intervenes when appropriate for clarification or correction.

Reviews the patient profile for allergies, interactions, and appropriateness before dispensing.

Checks dosage for appropriateness for pediatric, adult, and geriatric patients.

Carefully checks the work of Technicians before initialing labels. Directs Technicians to perform tasks appropriate to current priorities.

Participates in medication use evaluations, records Pharmacist interventions, and adverse drug events.

Provides support to the Pharmacy Management Team.

Build and enhance professional relationships with pharmacy staff and nursing staff.

Communicate effectively.

Provide pharmaceutical care to selected patients.

Develop drug therapy review programs and document their results.

Participate in Pharmacy and Therapeutics (PandT) Committee meetings.

Participate in the assessment and evaluation of professional staff.

Provide in-service education to medical, nursing and pharmacy staff.

Train and educate pharmacy staff to provide pharmaceutical care.

Participate in the publication of educational material directed at hospital health care professionals.
Position requires Bachelor’s degree in Pharmacy. Must have a valid State of California Pharmacist license.
Work hours are 4 days, 10 hour shifts.
