Looking for flexibility with your time?  We can offer you flex schedule in this quaint skilled nursing facility located 35 min away from the north side of Madison, just off Hwy 151.  As the in-house provider of therapy, we are able to provide consistency and stability to our staff (our parent company owns the facility – no chance of a lost contract here!).  This has lead to an established & tenured rehab team in the heart of Beaver Dam.  The gym is very large, and is easy for our patients to get to.  Situation just 2 blocks from the local hospital, the acuity level of our patients vary from the long term to the short term rehab patients.  We have our signature “GEM" program – Geriatric Enhanced Modalities:  Ultrasound, e-stim & diathermy along with several other specialty programs:  medically complex, falls and balance and a dementia program…  In addition, we make full use of the Nautilus equipment by following our unique FTF program – other wise known as a geriatric strength-training program.  These tools and programs allows us to focus on functional outcomes with our patients rather than just maintenance styled therapy.  Our caseload is comprised of 20% out patients, 20% dementia patients (we have an on-site ACU), and the rest split with neuro and ortho equally.  We have a full rehab team in place – ready to blend their skills with a tenured therapist, or willing to mentor and guide a new grad.

Requirements Physical Therapy Assistant
Carries out effective patient treatment plans to restore, compensate or adapt for loss of patient function, which are established by the licensed/registered therapist. Plans conform to federal, state and professional standards.

Qualifications
• Graduate from an accredited program for Physical Therapy Assistants or must possess and maintain a current license in the state of practice as required.
• Must possess problem-solving skills of the type and at a level necessary to accomplish the job.
• Must be able to communicate in English, both verbally and in writing.
• Must be capable of maintaining regular attendance.
• Must meet all local health regulations, pass post-offer drug test, and post-offer employment physical exam if required.
• Must be capable of performing essential job functions of this job, with or without reasonable accommodations.
• Must have own transportation to travel between sites as needed.
• Must possess computer skills of the type and at a level necessary to accomplish specific job duties and assignments.
• Must meet Clinical Competency requirements upon hire and as needed.
