Description Position available for Social Worker with Masters to make home visits in Shelby County. Will make assessments/evaluations and follow up visits on patients within Shelby County. Will be responsible for linkage to community resources.

Requirements Must be licensed MSW with valid TN license. Must have drivers license, auto insurance, and reliable transportation. Must be organized and able to manage time well. Must be able to work independently. Must possess good written and verbal skills. Knowledge of Medicare/Medicaid regulations a plus. Knowledge of available community resources in Shelby County required. Previous home care experience a plus.
