Admissions/Liason Home Care RN

The Hospice Home Care Nurse reports to the Admissions Team Manager.  He/she is responsible for identifying and thoroughly assessing patient/family care needs, and for initiating providing nursing and supportive care for the terminally ill patient and their families. This position requires travel to patient homes, assisted-living and skilled nursing facilities, and hospitals throughout Baltimore City and County, and Harford and Howard Counties to assess patients and discuss hospice care. This position would require 3 10-hour shifts, Tuesday, Thursday and Fridays. Days could be negotiable.

Education	Diploma, ADN and/or BSN from an accredited institution
Experience	Two years of nursing. Home Health or Hospice experience preferred but not required.
Skills	·         Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection ·         Strong time management skills ·         Excellent interpersonal and communication skills ·         Ability to be patient and family focused ·         Solid PC skills and comfortable with the use of EMR (Electronic Medical Record)
