Description This position is full time and has the option of working a weekend rotation OR every weekend. Functions with considerable autonomy in providing professional social work treatment and services: completes psychosocial and mental health assessments, counsels with patients and families, implements necessary treatment/case management plans, and ensures appropriate information and referral to facilitate a smooth transition to the most appropriate care environment. Serves as a hospital representative to local agencies, striving to enhance discharge planning, service delivery, promote professional respect and contribute to the smooth efficient running of the department. Gathers data for statistical reporting, productivity, quality improvement and other departmental studies. Provides designated clinical and staff assistance.

Requirements A Master's Degree in Social Work from an accredited school of social work. Two to three years experience in social work field, experience in health care setting preferred. Demonstrated leadership ability helpful. State licensing as applicable to degree held, must be minimally licensed at the graduate (LGSW) level. Ability to assess, treat and assist in planning for the age of the patients served, specifically: infants (0-1 year), childhood (13 mos-11 year), adolescents (12-17 years), adults (18-65 years), geriatric (Over 65 years).
