Description Blossom View in Sodus, NY is looking for the following candidates to join our team:

RN Case Manager - FT RN needed for management position. Case manager will be responsible for 20-25 residents. Current NYS License required. Previous Long Term Care experience, Case Management Skills, Leadership skills and MDS / Care Planning required.

RN, Restorative Coordinator - Immediate opening for an experienced RN to fulfill a key role in our clinical team. Qualified candidate will be responsible for implementation and ongoing maintenance of Restorative Nursing Program. Current New York RN License and previous experience in long term care a plus. MDS experience a plus.

Q/A Nurse, RN's - F/T or P/T Positions available for RN's. Previous quality assurance experience and long term care experience a plus. Current New York RN License required. Great for the retiree looking for a few days per week.

Team Leaders - LPNs who have a current NYS license and leadership experience directing team members who provide resident care including routine and skilled treatments and procedures. FT/PT and per diem positions available.

Certified Nursing Assistants - Current NYS certification required. Previous health care experience is highly desirable. FT/PT and per diem positions open. Must be available to work evening and night shifts.

Physical Therapist - Part Time- Physical Therapist needed to provide therapy services to patients in our outpatient therapy center. Possible evenings and Saturdays. Must be Graduate of an Accredited College or University with a Bachelor or Master Degree in Physical Therapy. Current NYS Licensure and Registration required.

Senior Physical Therapist - F/T candidate will head up rehabilitation therapied in both our in-patient and outpatient centers and be a key member in our clinical team. Must be familiar with MDS documentation and scheduling. Supervisory and electronic medical records experience a plus. Must be a graduate of an accredited program and have current licensure and registration in New York.

Speech Therapist - Experienced Speech Therapist needed to provide services to our residents. Must be Graduate of an Accredited Program and have current licensure and registration in New York.
