Position Overview:  Clinical /staff pharmacists fill orders for drugs, monitor patient drug therapies and provide drug information. The Clinical /staff pharmacists will work with physician to develop and maintain an effective formulary based on safety, efficacy and cost. The Clinical / staff pharmacist will be responsible for rounding with medical staff, participating in education of pharmacy students, nurses, physicians, other allied health professionals and the community. The Clinical / staff pharmacist will supervise and direct supportive personnel. In the absence of the pharmacy manager, a Clinical / staff pharmacist may be required to assume the essential responsibilities and perform the duties of the pharmacy manager.

Responsibilities:  A. Prepares and dispenses drug orders per physician request according to established policies, procedures and protocols.1. Interprets drug orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete pharmacy patient medication profiles.2. Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products accurately.3. Issues controlled substances to patient care areas and maintains records as required by law.B. Contributes to the quality and effective operation of the department1. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.2. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner. 3. Completes all assigned medication storage area inspections in a timely manner. Identifies and replaces outdated and unusable drugs.4. Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window. 5. Keeps pharmacy areas and equipment clean, neat and well organized. 6. Oversees operation of the pharmacy in the absence of the pharmacy manager as necessary. C. Maintains competency required for current job title/position.1. Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files. 2. Attends pharmacy staff meetings.3. Attends orientation, education and training programs. Reviews literature and other materials pertinent to the practice of pharmacy.4. Completes all competency/skills assessment requirements(see attached competency assessment/skills list)D. Performs other duties as assigned by supervisor.Clinical Performance CriteriaII. Clinical Duties/ResponsibilitiesA. Ensures safe, appropriate, cost effective drug therapies for patients according to established policies, procedures and protocols.1. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. 2. Participates in patient rounding and takes responsibility for necessary follow up regarding patient therapy3. Review/interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed.4. Reads, extracts, and interprets information in patient charts accurately.5. Detects and reports suspected adverse drug reactions accurately and in a timely manner.6. Sustains the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.7. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.8. Provides accurate, adequate and timely drug information to the hospital’s professional staff. 9. Provides education to pharmacy students, nurses, pharmacists, physician, patients, family and community.10. Documents all clinical activities and interventions accurately and completely.11. Participates in the quality improvement and medication use review activities of the department. Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned. 12. Participates in the development and presentation of orientation, education and training programs for the medical staff, nursing service and other hospital staff.13. Conducts designated interventions as defined the departments Clinical Intervention Program (IV to PO, Renal Dosing, etc.)1. Is punctual and dependable; reports to work as scheduled. Fulfills on-call obligations per pre-arranged schedule. Absenteeism and tardiness are within policy guidelines. 2. Maintains a neat, professional, well-groomed appearance. Observes pharmacy uniform dress code. Wears identification badge. 3. Performs work within specified time frames. Adapts positively to frequent interruptions and changes in workload and/or work schedule.4. Provides courteous, cooperative and timely service to patients, visitors and staff. Demonstrates good verbal and written communication.5. Works cooperatively with hospital and pharmacy staff. Voices concerns and suggestions to appropriate persons in a positive manner.6. Demonstrates sound professional judgment consistent with clinical/academic background.7. Maintains strict confidentiality of patients, guests and employee information.8. Fosters a team environment by providing orientation and training to new team members. Assists co-workers in tasks, as time permits.9. Adheres to hospital and departmental policies and procedures. Complies with all requirements related to risk management, safety, security, fire and infection control. Complies with all applicable Federal and State laws, rules and regulations.Organizational Standards 1. Requirements Education: Graduate of an ACPE accredited School of Pharmacy with a Doctor of PharmacyLicenses: Currently licensed to practice pharmacy in the state of Delaware or reciprocation eligibleExperience: 2 years of hospital pharmacy, a residency preferred.
