Job Description:  Summary:The purpose of this position is to prepare pharmaceutical prescriptions in an accurate and timely manner, and to provide the related, distributive and clinical pharmacy activities to provide services to home infusion therapy patients.Major Responsibilities:Coordinates and facilitates new referrals/admissions through collaboration with nursing, reimbursement, operations and sterile productsReviews, receives/verifies all new patient orders and documents the same on the respective order formAssists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapyConducts initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problem or needsConducts and revises the patient plan of careAssists the physician and nursing staff in setting therapeutic objectives for drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.Communicates the results of therapeutic drug monitoring activities to the physician, nursing and other appropriate persons, makes appropriate recommendations which are designed to enhance the outcomes associated with drug therapy.Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals.Prepares sterile admixtures, dispenses all pharmaceuticals and related supplies and equipment in accordance with federal, state and local laws and standards of practice which governs the profession of pharmacy.Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.Programs infusion control devices as indicatedParticipates in departmental QA activitiesAttends designated department meetings as scheduledAdheres to all CCS Policies, Procedures, and Standards of Conduct covered during initial orientation and referenced in branch handbooks, guides and manuals.Attends mandatory annual inservices that are required by current position heldReports to the appropriate manager(s) any known or suspected violations of policies and procedures, regulations or standards of conductPerforms other duties as assigned
Requirements A Bachelor of Science in Pharmacy is required; A Doctor of Pharmacy or a Masters of Science in Pharmacy is preferred. The candidate must be currently licensed as a registered pharmacist in state of employment; must possess: a minimum of two years experience in an acute-care setting or one year recent experience in home infusion therapy; experience and a strong knowledge base in intravenous admixture services and intravenous drug therapy; the capability of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals; the ability to work effectively with others to provide safe and effective drug therapy; the ability to problem solve and supervise technical and other support staff; flexilility with work schedule; excellent verbal and written communication skills; good organizational and time-management skills and must be dependable in attendance and job performance.
