Community Health Worker

Aids YVFWC CHS clients in obtaining needed social and health services in an integrated and coordinated manner. Directs and carries out Infant Case Manager (ICM) care plans.

Qualifications

Education: Bachelor’s Degree in a social service field such as social work, behavioral sciences, psychology, child development or the like.

Experience: One year of experience working in community social services, public health services, crisis intervention, outreach and linkage programs or other related fields.

Licenses/Certificates/Registration: Current Washington State driver’s license and proof of automobile liability insurance coverage.

Knowledge/Skills/Abilities Required: Bilingual in English/Spanish with demonstrated ability to communicate at level 10 on the ALTA language proficiency scale; excellent communication skills; knowledge of resources available in the community; writing skills for documentation in English; and knowledge of medical terminology.
