Responsibilities:
* Fill out admission forms including medical history, assessment of the patient’s physical and emotional status, level of competency, home environment, safety factors, family or household member’s ability to assist with the care and needs of the resident and determine what teaching will be required.

Supervise and direct the care given by the LPN, HHA or other service providers and evaluate such care in accordance with Agency, government and third party payer guidelines and regulations.

Participate in on-call rotating schedule and provide backup assistance as requested on weekends and holidays.

Confirm the scheduling of visits with the DON to coordinate necessary visits with other personnel.

Requirements:
* MUST HAVE HOME HEALTH EXPERIENCE
* Must be registered nurse in good standing in the state of Florida.

Minimum 1-2 years experience as a registered nurse.

Must have experience in a Medicare home health.

OASIS and coding exp a plus.

Computer skills a plus.
