Job Duties:

• The Case Manager is responsible for the care given to a group of patients. This includes implementation of Case Management System, assessment of patient needs, development of patient care plans, scheduling of patient and team assignments, administration of nursing care, evaluation of nursing care and supervision of non-professionals and para-professionals in the delivery of patient care in the home care setting while maintaining productivity standards.
• The Case Manager has overall responsibility for the successful management of a patients' case.
• Successful management will include positively impacting: Clinical outcomes, patient care plan goals, appropriate utilization of the agency’s financial and human resources, and patient satisfaction.
