Job Description 

Yoh Health Care has a direct hire opening for a Pharmacist to join our customer in Buffalo, NY.

 

Job Duties:

 

  • Looking for a staff pharmacist to clinically evaluate medication orders using the patient profiles and the system's on-line medical record to assure safe therapy for our customer's patients.
  • Pharmacist will be working together with other health professionals regarding any medication medication-related problems.
  • The pharmacists are called upon to provide drug information to health care providers and patients and they participate in patient care rounds/therapeutic monitoring as assigned.
  • Staff are involved daily in antibiotic dosing and monitoring.
  • Teaching activities include pharmacy resident and students.
  • Medications are dispensed to inpatients and LTC patients with the help of automated dispensing systems and Pyxis Connect.
 


Job Requirements 

  • B.S. Degree in Pharmacy or Pharm D required.
  • Previous hospital/home care experience preferred; residency in hospital pharmacy preferred.
  • NYS Licensure and current NYS Registration.
