Job Description

VNS is the second largest Non-Profit Home Care agency and has recently gained national recognition as one of the Elite Home Care providers for our delivery of quality care to patients in their home environment.

 

We currently have openings for Full Time, Part Time and Per Visit Occupational Therapist in Bridgeport.

 

Daily responsibilities include:

  • The Occupational Therapist is responsible for evaluating patient's level of function and developing an appropriate plan of care for patients in their home environment
  • Ensures that Branch and Agency standards, policies, procedures and protocols, and goals and objectives are achieved.
  • Delegates appropriate patient care responsibilities to C.O.T.A and Home Health Aides, patient, and family members in the care of the patient
  • Responsible for accurate completion of Oasis documentation
