Assistant Clinical Supervisor For Home Care -  Hartford Region for home care teams; stationed in Hartford, will primarily cover greater Hartford area teams but will occasionally assist our Northern CT office and Glastonbury as needed.  Will precept and mentor new RNs in case management, OASIS admissions, and other required home care skills.  Will utilize problem solving and interpersonal skills.  Must be flexible, as direction for this position will be determined by current needs as set by the Clinical Supervisors and Regional Clinical Director.

Requires valid CT RN license.   MUST have BSN, or be willing to complete mandatory community health courses within the next year.  Must have minimum two years recent (within 2/3 years)licensed home care experience working with admissions, OASIS, Medicare/Medicaid adult/geriatric patients in med/surg settings (wound care, diabetes, post-op, COPD, CHF, etc.)  AND

At least one (1)  year supervisory experience, preferably in home care but can be in acute or sub-acute care.  

MUST have good computer skills, preferably working with home care laptop program.
