Direct Care Activities Worker / Day Program – Non-Profit

Will provide support to people with severe and multiple disabilities.  Assist in the development and implementation of individuals support plans as part of a day habilitation program.

1. Understand and meet the care needs of Individuals served.
2. Understands difference in ISP, action plan, service plan, etc. and implements with assistance and queuing.
3. Ensures action plans and service plans implemented and documented.
4. Ability to care for personal needs such as brief changes as well as life skill and hygiene assistance.
5. Knowledgeable of all required reporting documents:

   • ID notes

   • Vital sheets

   • Nursing notes

   • Communication logs

   • Time sheets

   • Attendance

   • Seizure reports

   • RAD forms

   • Incident reports

EDUCATION AND EXPERIENCE

• High School Diploma or GED
• (1) year experience working with people with developmental disabilities.
• Must have a Drivers License and a clean DMV History as well as meeting the insurance requirements to drive a 15 passenger van; age 25 or above.
• Certified Nurse Assistant a PLUS.
