Job Description
This Case Manager position, in collaboration with the physician and as a member of the healthcare team, is responsible for ensuring the patient's progression through the continuum of care in a manner that achieves the desired clinical and financial outcomes. Participates in the coordination and direction of care for patients from admission through discharge. The Case Manager must be prepared to address the issues of health assessment and appropriate intervention. The role requires the nurse to have an understanding of financial reimbursements and third party payments. Coordinates and promotes an advanced level of nursing care. Highly desire case management experience.
Job Requirements

Basic Qualifications (required) Required Certification/Registration: Current State of California RN license. Availability to work days and rotate weekends.

 

Minimum Qualifications (required) 3 years of recent acute care clinical case management experience. American Heart Association BLS required by date of hire.

 

Preferred Qualifications Case Manager experience strongly preferred. Current ACLS Provider and/or Certification in Case Management preferred. Home healthcare, discharge planning, or prior experience in utilization review are preferred. Knowledge of various reimbursement methodologies.
