Before any applicant is able to be considered, you will have to provide the following:
- Valid AZ Nursing License ( or Compact State)
- Valid CPR/BLS
- Valid ACLS and other certification, if applicable
- Drivers License
- Social Security card
- Current physical (stating that you are fit to work without restrictions)
- Current TB test (and/or Chest X-ray)
- As well as proof of immunizations of Rubella, Rubeola, Mumps, Varicella, Hep B (or you can sign or declination form)
Job Requirements
Mandatory:
- Must be familiar with Ventilators & Trachs
- Must be able to work well with higher acuity patients
- Must have 2 years of experience in one or more of the following units- Tele, M/S, ICU
- ACLS Cerification
- A graduate from an accredited registered nursing program
- Licensure as a Registered Nurse in the state of Arizona.
